This webinar is for anyone who has been diagnosed with breast cancer and wants support after treatment. You will learn about survivorship care plans and how Oncolink’s® OncoLife Survivorship Care Plan can help you understand life after a breast cancer diagnosis. The plan can guide your follow-up care, help you manage long-term and late effects, and support healthy living after treatment.
Christina Bach, MBE, LCSW, OSW-C, FAOSW, Oncology Social Worker and Content Specialist at OncoLink®, will provide information about survivorship, including information for those with metastatic disease who may be living with chronic cancer management. She will then show you step by step how to find and personalize an OncoLink® OncoLife Survivorship Care Plan during a live demonstration. She will highlight how to use your Survivorship Care Plan to help communicate with your healthcare providers. There will also be time for questions, so you can get answers that are important to you.
